Referral Pathways
Simple, supportive referrals for clients, families, and service providers.
KCSA makes the referral process easier by helping clients, families, support coordinators, case managers, health professionals, and service providers connect with the right support.
Making referrals easier and more supportive
Referring someone for support should feel clear, respectful, and easy to follow. KCSA works with clients, families, carers, service providers, and professionals to help understand needs, gather information, and guide the next steps.
Whether the referral is for aged care, NDIS support, settlement support, language access, transport, or advocacy, our team can help explain what information is needed and how support can begin.
A clear pathway from enquiry to support
We understand that every person’s situation is different. Some clients may need urgent support, some may need help understanding services, and others may need support with forms, assessments, or communication.
KCSA provides a warm and culturally responsive referral process so clients and families feel informed, respected, and supported from the first contact.
How Referrals Work
-

1) Initial Enquiry
Contact KCSA by phone, email, online form, or through a service provider. We will listen to the situation and identify what support may be needed.
-

2) Information Gathering
We collect key details such as the client’s needs, preferred language, funding type, support goals, risks, and any urgent concerns.
-

3) Service Matching
Our team reviews the referral and matches the client with suitable KCSA services, workers, or referral options.
-

4) Client & Family Communication
We keep the client, family, carer, or referrer updated and explain the next steps in a clear and respectful way.
-

5) Assessment & Planning
Where needed, we organise care discussions, support planning, home safety checks, or service coordination before supports begin.
-

6) Support Commencement
Once everything is confirmed, KCSA helps arrange the first visit, introduces the support worker or staff member, and checks that support is working well.
